If you are a cleared volunteer of St. Jude Catholic Church
and would like to be placed on the cleared drivers list,
please complete the "Motor
Vehicle Release Statement" and fax it to ACCUTRAK
Investigative Services. There fax number is listed on
the form. They will notify me of your clearance and you
will be added to our cleared driver list. We will also need
a copies of your current insurance card an valid driver's
license. There will be no fee on your part for the MVR check.
